Care Managers work with patients and the care team during an admission to help manage all aspects of care. A specialist in insurance, discharge planning, community resources and social work, the Care Manager coordinates a patient's special needs. When the need for acute care is no longer necessary, patients will be discharged to home or to a more appropriate level of care (nursing home, rehabilitation, home care or other discharge needs). The Care Manager works as a part of the care team to make this transition as smooth as possible.
