Advance Directives and Living Wills

Advance Directives and Living Wills are a means for you to tell your healthcare providers about the care you wish to receive — or not to receive — should you ever become unable to tell them of your wishes. Advance Directives forms are available at the registration area or by calling the patient representative at 570-621-4797.

Deciding About Cardiopulmonary Resuscitation

A patient's right to decide about treatment also includes the right to decide about Cardiopulmonary Resuscitation (CPR). CPR is emergency treatment to restart the heart and lungs when breathing or circulation stops. Sometimes doctors and patients decide in advance that CPR should not be provided, and the doctor gives an order not to resuscitate (DNR). If a patient's mental condition prevents him or her from deciding about CPR, someone he or she appoints, his or her family members or others close to the patient can decide.
